Remember, this form must be submitted to Miss Silfia's e-mail before june 22nd, 2010. If you do not submit it or you forget to submit it, you will not be allowed to join the final exam of Reading IV. So, make sure to send it in time, otherwise you will lose your exam!

Make sure you inform your fellas to fill out the form completely, and send it back to me before your final exam week. A and B,before june 22, and C and D before June 24.
